Applications can be submitted three ways:

• Download, print and complete paper application here
• Mail your completed paper application to: MERITS, P.O. Box 210040, Tucson AZ 85721
• Fax your completed paper application with “Attention to MERITS ” to:  520-626-0096
• Drop off your completed paper application at the Front Desk in the Robert L. Nugent Bldg., Main Level

You may also obtain applications and additional information by mail or from the MERITS staff. Visit our offices in the Robert L. Nugent Building or call us at 520-621-3093.
